The Client
A U.S. medical billing company that manages revenue cycle work for outpatient specialty practices. One of its practice clients is a diagnostics group with several locations across New York City, generating a steady stream of requests from attorneys, insurers, and other providers asking for copies of patient records.
The Challenge
Medical records requests are unglamorous, PHI-heavy, and easy to under-resource. Each one requires locating the right patient chart across the practice's locations, pulling the specific notes or documents requested, logging what was sent and to whom, and getting a response back inside a defined turnaround, all without a mistake in identifying the right patient or record. None of that is billing work in the clean-claim-rate sense, but it still consumes billing-staff time whenever it lands on the same desk as claims and A/R follow-up.
As request volume grew, the billing company needed someone who could own this queue full time, work directly inside the practice's own EHR and practice-management systems the way an in-house employee would, and do it under the same compliance standard the practice itself operates under. That meant granting an outside team real access into their environment, not a data export, which raises the bar on how that access gets set up and governed.
What RCM Staff Did
RCM Staff placed a dedicated Medical Records Request Specialist with the billing company, staffed and managed to work inside the client's existing tools and workflow rather than a separate system of our own.
- Compliance before access. A signed Business Associate Agreement was in place, and the assigned specialist completed HIPAA training, before any credentials to the practice's systems were issued.
- Access built for the client's environment. The billing company provisioned dedicated logins to its EHR and practice-management platform along with a secure connection into its network, and RCM Staff worked strictly inside those client-approved systems rather than routing PHI through outside tools.
- A documented intake-to-fulfillment workflow. Incoming requests are triaged from a dedicated inbox, matched to the correct patient chart across the practice's multiple locations, and the applicable notes or documents are located and prepared for release.
- A record of every request. Each fulfillment is logged to the client's own tracking system, so the billing company has full visibility into what went out, to whom, and when.
- A two-week structured ramp-up, consistent with how RCM Staff brings on every new augmented role, so the specialist was working independently against the client's SOP well inside the first month.

The billing company did not need a new vendor to take this work off its plate. It needed one more trained person, working the way its own staff already work, inside systems it already controls.
